Here is additional information from Antti: “that Western validation refers to a few studies on the use of pepsinogen and H. pylori, not GastroPanel as such” Wrong. Quote from the paper, p. 4: “The revised ABC method, marketed as GastroPanel, adds G-17 to improve specificity and to allow topographic characterization of atrophy.”[23] “At the GastroPanel threshold of PGR ≤ 5.3, sensitivity is 51–59% and specificity 61–66%. Combining a low PGR with H. pylori-negative status yields adjusted ORs of 3.36” So the paper:  Mentions GastroPanel by name as a commercial product - the only one in the entire review States its PGR threshold of 5.3 (old) and compares it to the new PGR of 6.15 Reports the OR figures at the GastroPanel threshold  “Closest to deployment” - Table 7 and the final conclusion of the paper state: “In Western health systems where population endoscopic screening is not cost-effective, one-time serum pepsinogen testing with H. pylori serology… is the strategy for which the evidence is strongest: it has the only large Western validation dataset [PLCO], contemporary supporting European data, prospective evaluation in two cohorts, and a favorable modeled ICER.” This is the “closest to deployment” - meaning Yale states that this strategy (pepsinogen + HP serology = GastroPanel) is the ONLY one that has a large Western validation [PLCO trial USA n=∼150k] + 2 prospective cohorts + cost-effectiveness below the NHS threshold. Liquid biopsy and AI DO NOT have Western validation. I

Thanks for the reply, Antti. Here is additional information: “I haven’t come across any public information” True, it’s not public because the NICE submission is confidential until 2027. But: • Homerton 2025 (UK, n=324, multi-ethnic London) is referenced in the British Society of Gastroenterology (BSG) 3/2026 guideline  • Kappa 2025 OYS (90% of gastroscopies avoidable) is Biohit’s own Finnish data  • When NICE updates in 2027, it will look at BSG + Homerton + Yale 2026 review. That’s why this Yale paper is important – NICE cites it. And the article does mention GastroPanel by name twice on p.4: “marketed as GastroPanel” and gives it a PGR ≤5.3 threshold with OR 3.36/2.25. Western validation refers to PLCO + EU data, which is precisely GastroPanel’s strategy (PGR + G-17 + HP). “Closest to deployment” is the Table 7 summary – serology is the only one with Western data. Could you re-check the section “revised ABC method, marketed as GastroPanel”?
